AIA Services New Zealand Limited (“AIA”) collects, uses, discloses and handles personal information about you for purposes which include the assessment of your eligibility for membership in the AIA Vitality Program, the administration and provision of services and marketing communications that may be of interest to you, and for communications in relation to the AIA Vitality program.

Personal information that you provide will be used and disclosed in accordance with the terms and conditions of the AIA Vitality Program and will be collected, used, stored and disclosed in accordance with AIA’s privacy statement.

By submitting my registration, I confirm that:

I have read and agree to:

the Terms and Conditions of the AIA Vitality Program; and
the AIA Terms of Use; and
I acknowledge that these terms may be amended, added to or replaced by AIA from time to time and that I should check the AIA Vitality website regularly for the most up-to-date version.

I provide authority and consent for AIA to use and disclose my personal information (other than health information) to AIA Australia, AIA Group, AIA Vitality Company Limited (HK), Discovery Holdings Limited (SA)* and their related entities, subsidiaries, affiliates and partners to administer the AIA Vitality Program, including to provide products, benefits and services related to my AIA Vitality membership.

Electronic communications must be regularly checked and it is my responsibility to ensure that I provide AIA an up-to-date, unblocked and unfiltered email address and that email from AIA is not filtered. I acknowledge that hard copies of AIA Vitality information may not be provided but that they may be sent at AIA's discretion.

  any health information that the receiving party receives arising out of or in connection with the AIA Vitality Program solely for the purposes of administering the AIA Vitality Program;
use any health information that the receiving party receives arising out of or in connection with the AIA Vitality Program solely for the purposes of administering the AIA Vitality Program; and
disclose any health information that the receiving party receives arising out of or in connection with the AIA Vitality Program to AIA Group, AIA Vitality Company Limited (HK), Discovery Holdings Limited (SA) and their related entities, subsidiaries, affiliates and partners solely in an anonymised, aggregated manner and in such a way that the health information is not reasonably capable of being de-anonymised by these entities.
