Earn 1,000 Points for when you have received both initial doses of the COVID-19 vaccination (dose 1 & dose 2), or for your booster vaccination.

To be eligible for points:

  •  

Vaccinations must have been completed within the last 180 days. Points will only be awarded after you have received all the doses required to be fully vaccinated (dose 1 & dose 2), or for your booster vaccination.

  •  

Please upload a photo of the approved COVID-19 vaccine record card, confirmation from your GP or health professional or a completed AIA Vitality Health Check & Screening form.

  •  

Only submit once you've received both doses (dose 1 & dose 2) or the booster vaccination.
